Kerb stone come away from path

Kerb stone has come away from the path again, this was reported last year, fixed and is now broken again, all the cement that was filled in has chipped and broken, stone its self has broken. the kerb stone has also raised from the kerb making it a dangerous trip hazzard and has lifted from the slot it sits in.
