Sunk tarmac creating trip hazard

Sent to West Northamptonshire Council less than a minute later. ref: 9158837.
Tarmac around a access cover has sunk leaving the concrete pad as a trip hazard, heigh of pad is approx 1 inch higher than the surrounding tarmac, per pictures.
